 RAINTOWN DOG TRAINING AGREEMENT

1.     I have voluntarily employed Raintown Dog Training Inc. (“Raintown”) to assist me in the training of my dog(s).

2.     I understand that I am responsible for payment in full regardless of whether I choose to complete the training program. Cancellations with refund requests made up to 48 hours before my scheduled private sessions and/or series of classes begin will be refunded, minus a $10 fee. Missed classes cannot be rescheduled or refunded.

3. I understand that Raintown allows up to one rescheduled private session if it is rescheduled up to 48 hours before my scheduled session.

4. I understand that if, in group classes, my dog shows signs of over-arousal, reactivity, or any other behavior deemed unfit by Raintown for classes, I will be asked to drop out of this round. Raintown may use my remaining class credits to put towards a private session, depending on credits remaining and availability of Raintown trainer.

5.     I understand that my dog must be up-to-date with all vaccines (including distemper and parvovirus) in order to attend training sessions with Raintown.  For Puppy Kindergarten, a minimum of the first round of vaccines must have been administered for the dog to be allowed to attend class.  I agree to provide Raintown with an up-to-date copy of my dog’s vaccine records.

6.     Raintown will make every reasonable effort to help me attain training goals, but makes no guarantees of performance on the part of myself or dog as a result of providing professional training services.

7.     Raintown will endeavor to create as safe an environment as possible for the training of my dog and will offer only sound, safe, and responsible training and training instructions. However, to the extent that Raintown is insured for any unintentional or negligent errors, omissions, or incorrect assertions, Raintown will be responsible for any such acts or omissions, but only to the extent of such insurance. I understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others. I represent and warrant that I have provided Raintown with full and complete, accurate information regarding any bite history and similar incidents or hazardous tendencies of my dog, and that I will update that information if it changes. I understand that I am and will remain responsible for the actions of my dog at all times, and I hereby agree to indemnify, release, and hold harmless Raintown of any and all claims, whether made by myself or any third party, of injury, expense, costs, or damages caused by my dog. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.

8.     I authorize Raintown to enter my home during agreed upon days and hours for the purpose of training my dog. I authorize Raintown to take my dog off my property during the agreed upon days and hours for the purpose of training my dog.

9.     I authorize emergency medical care to be provided for my dog(s) by my veterinarian, or an appropriate alternate to be determined by Raintown in the event that my regular veterinarian is not available or that closer care is required. I will reimburse Raintown for any charges related to emergency care, including office visits, procedures, medications, surgeries, etc.

10.     I grant permission to Raintown the irrevocable and unrestricted right to use, publish and broadcast my photograph(s), likeness and voice, including via video, for trade, advertising, and any other purpose and in any manner and medium; and to alter the same without restriction. I hereby release Raintown from all claims and liability relating to said photograph(s), video(s), likeness and voice.

11. I understand that once I have paid for private sessions and/or classes, I am responsible for scheduling and completing these sessions and/or classes within three months of my payment date. Any sessions and/or classes not scheduled and completed after three months will be forfeited.

This contract, together with language expressly incorporated into it in writing, is the full and complete agreement between myself and Raintown.
